Does massage actually help recovery, or does it just feel good?

One of massage's most repeated claims collapsed under a meta-analysis. Another one held up better than anyone expected. The honest picture is more interesting than either side admits.

Yes for soreness, no for most of the rest, and the famous stress-hormone claim does not survive a close look.

The strongest finding is that massage genuinely reduces delayed onset muscle soreness, the ache that shows up a day or two after hard training. Multiple meta-analyses agree on that. What it does not reliably do is improve your actual performance, flush lactic acid, or reliably drop your cortisol.

Feeling good is not a consolation prize here. It is most of the value, and it is worth being clear-eyed about why.

What does massage genuinely do?

It reduces soreness, and that is the best-supported effect it has.

A meta-analysis by Dupuy and colleagues (Frontiers in Physiology, 2018) compared recovery methods across dozens of studies and found massage came out best for reducing delayed onset muscle soreness and markers of inflammation. That is a real, replicated result, not a marketing line.

If you have trained hard and you ache, a massage will likely make you ache less. That is a genuinely useful thing, especially if soreness is what stops you training again on schedule.

Does it improve performance?

Barely, if at all.

Poppendieck and colleagues (Sports Medicine, 2016) reviewed the studies on massage and subsequent athletic performance and found the effects small and inconsistent. Some studies show a tiny benefit, some show nothing, and the better-controlled the study, the smaller the effect tends to get.

So if you are booking a massage expecting to sprint faster or lift more the next day, the evidence does not support that. It supports you feeling less wrecked, which may indirectly help you train more consistently. That is not nothing, but it is a different claim.

This is a recurring pattern in recovery research: things that make you feel better often do not measurably make you perform better.

What happened to the cortisol claim?

It is the most cited massage statistic, and it did not hold up.

You will still see studios claim massage cuts cortisol, your main stress hormone, by around 31 percent. That figure traces back to a review by Tiffany Field and colleagues (2005), which pooled a set of small studies.

Christopher Moyer and colleagues (2011) went back and did a proper meta-analysis of massage and cortisol. They found that single-session massage produces at most a very small reduction, nothing like 31 percent, and that the effect is not reliably distinguishable from zero. Their conclusion was blunt: the cortisol reduction claim is not supported by the evidence.

That matters, because “lowers your stress hormone” is the mechanism the whole relaxation story leans on. The relaxation is real. The hormonal explanation for it is largely unearned.

What about flushing lactic acid?

That one was never right.

The idea that massage squeezes lactic acid out of your muscles is folk physiology. Lactate clears from your blood within roughly an hour of exercise on its own, and it is not the cause of next-day soreness anyway. Soreness comes from microscopic muscle damage and the inflammatory response to it.

There is even evidence that massage slightly reduces blood flow rather than increasing it during the session, which is the opposite of the flushing story.

If a therapist tells you they are flushing your lactic acid, they are a nice person repeating something they were taught. It does not make the massage worthless. It just is not what is happening.

So is it worth the money?

Yes, if you buy it for what it actually delivers.

You are buying less soreness, an hour of genuine relaxation, the effect of skilled touch, and a period where nobody can reach you. Those are real goods. The pain-and-tension relief people report is consistent even where the biochemical explanations fall apart.

Book it because you want it. That is a sufficient reason, and it is an honest one.

When is massage the wrong call?

When you need a diagnosis rather than an hour on a table.

Persistent pain, pain with numbness, tingling or weakness, or pain that has changed in character are reasons to see a doctor or physiotherapist first. Massage is a good comfort intervention and a poor diagnostic one.

The bottom line

Massage reliably reduces muscle soreness, does little measurable for performance, does not flush lactic acid, and the 31 percent cortisol figure did not survive a proper meta-analysis.

It still works on the thing most people actually want from it: you feel considerably better afterwards. Just do not let anyone sell you a mechanism they cannot support.
